Mitral Valve Leaflet Remodeling After Myocardial Infarction

چکیده

It is a matter of force. It is a balance of valve closing force and tethering force that affects mitral valve coaptation. Ischemic mitral regurgitation develops secondary to left ventricular dysfunction generally with structurally normal leaflets. However, it seems it is not simply a matter of force when we see the fact that valve leaflets histologically change after myocardial infarction. The leaflets can stretch, can grow in area, and can increase in thickness. Recent studies have shown evidence of leaflet remodeling and have proposed new insights from the biological reaction of the leaflet. Some specific biological markers are activated in the ischemic environment and cause histopathologic changes in the leaflet. Thus, it is not just about the force, and we cannot ignore the leaflet pathology and function when we consider the mechanism of ischemic mitral regurgitation. Mitral valve complex is becoming more complex.

Mitral leaflet adaptation to ventricular remodeling: prospective changes in a model of ischemic mitral regurgitation.

BACKGROUND Ischemic mitral regurgitation is caused by systolic traction on the mitral leaflets related to ventricular distortion. Little is known about how chronic tethering affects leaflet area, in part because it cannot be measured repeatedly in situ. Recently, a new method for 3D echocardiographic measurement of mitral leaflet area was developed and validated in vivo against sheep valves, la...

یک مورد نادر پارگی سر عضله پاپیلاری قدامی طرفی بدون انفارکتوس حاد میوکارد و یا بیماری عروق کرونری

A rare complication of AMI is papillary muscle rupture (PMR), occurring in 1% of patients with acute myocardial infarction (AMI). Mitral regurgitation related to PMR occurs at a median of 1 day (ranging from 1-14 days) after onset of myocardial infarction. PMR tends to occur in those having a first time AMI and those without extensive collateral circulation. PMR involves posteromedial papillary...

Mitral valve prolapse in patients with coronary artery disease. Echocardiographic-angiographic correlation.

Echocardiography was performed in 25 consecutive patients with angina pectoris and angiographically demonstrable coronary artery disease. Left ventricular echograms detected late or pansystolic mitral valve bowing suggesting of mitral valve proplapse in 6/25 (24%). Left ventricular angiography showed prolapse of the posterior mitral leaflet in 15/25 (60%), including 5 detected by echocardiograp...

Massive mitral regurgitation secondary to acute ischemic papillary muscle rupture: the role of echocardiography.

Papillary muscle rupture is an infrequent but often fatal mechanical complication of acute myocardial infarction (AMI). We report the case of an AMI complicated by the development of an abrupt cardiogenic shock due to the rupture of the head of the postero-medial papillary muscle with echocardiographic demonstration of severe mitral regurgitation due to flail posterior mitral valve leaflet. Aft...

Valvular Heart Disease Comprehensive Annular and Subvalvular Repair of Chronic Ischemic Mitral Regurgitation Improves Long-Term Results With the Least Ventricular Remodeling

Background—Undersized ring annuloplasty for ischemic mitral regurgitation (MR) is associated with variable results and 30% MR recurrence. We tested whether subvalvular repair by severing second-order mitral chordae can improve annuloplasty by reducing papillary muscle tethering. Methods and Results—Posterolateral myocardial infarction known to produce chronic remodeling and MR was created in 28...
